چکیده

Advanced diabetic retinopathy with vitreous hemorrhage or tractional retinal detachment (TDR) is an important cause of severe visual loss in patients with proliferative diabetic retinopathy (PDR), frequently representing an indication for pars plana vitrectomy in diabetic patients. In these cases, surgery is not free from complications such as intraoperative vitreous hemorrhage and retinal rupture. Also, during the postoperative period, these patients may suffer untoward events such as intraocular rebleeding and neovascular glaucoma. Antiangiogenic drugs have been extensively used for the treatment of chorioretinal vascular disorders. The intravitreal use of bevacizumab, ranibizumab and pegaptanib has shown positive effects on diabetic retinopathy. The inhibition of vascular endothelial growth factor (VEGF) by these drugs can induce regression of diabetic neovascular retinopathy and combat exudative phenomena, thus being useful for the preoperative management of these patients. In the present article we review the use of antiangiogenic drugs as adjuvants in pars plana vitrectomy for the treatment of complications of diabetic retinopathy.

What do Cochrane systematic reviews say about diabetic retinopathy?

CONTEXT AND OBJECTIVE: Diabetic retinopathy is a disease caused by increased permeability of retinal vessels. Its incidence and prevalence have been increasing due to urbanization, greater life expectancy and the habits of modern life. Its onset is insidious and it may lead to blindness in 75% of individuals who have been diabetic for more than 20 years. The aim here was to evaluate the evidenc...

The Role of Angiogenesis in the Development of Proliferative Diabetic Retinopathy: Impact of Intravitreal Anti-VEGF Treatment

Although cellular and molecular bases of proliferative diabetic retinopathy are only partially understood, it is evident that this complication of diabetes is characterized by the formation of new vessels inside the retina showing abnormal architecture and permeability. This process, if not controlled by selective laser photocoagulation, leads to irreversible retinal damages and loss of vision....

Clinical biomarkers and molecular basis for optimized treatment of diabetic retinopathy: current status and future prospects

Diabetic retinopathy is a highly specific microvascular complication of diabetes and a leading cause of blindness worldwide. It is triggered by hyperglycemia which causes increased oxidative stress leading to an adaptive inflammatory assault to the neuroretinal tissue and microvasculature. Prolonged hyperglycemia causes increased polyol pathway flux, increased formation of advanced glycation en...
